Grantmaking Guidelines

Overview

Community Hospital of the Monterey Peninsula, on behalf of Montage Health, provides grants from the Community Benefit Fund to registered nonprofits, schools, faith-based organizations, or fiscally sponsored agencies in support of health and wellness services within Monterey County.

We will prioritize projects that align with Montage Health’s mission and address issues identified in our service area’s 2022 Community Health Needs Assessment. We recommend aligning your project with one of the following priorities:

  • Access to Healthcare Services – programs to support the ability to obtain healthcare services such as prevention, diagnosis, treatment, and management of diseases, illness, disorders, and other health-impacting conditions.
  • Diabetes – programs that facilitate the knowledge, skills, and ability necessary for diabetes self-care, prevention, and awareness.
  • Mental Health – programs that directly support emotional and psychological well-being.

Grants are typically single-year commitments, from $1,000 to $10,000. Awards of any amount will require an outcome report within 12 months of the award date. Sponsorships require proof of fulfillment through photos or other supporting documents.


Application and Review Process

Grants are reviewed through two separate application processes through our online application portal, here.

1. Program Support Grants - Reviewed on a monthly basis.

2. Sponsorship Grants - Reviewed on a rolling basis (1-2 weeks). Please be in touch early enough for logo deadlines.

As of April 1, 2023, mailed or emailed proposals will no longer be accepted. To download a list of application questions and required documents in preparing for your grant application, please email grants@montagehealth.org.


Keep In Mind

Community Hospital, a part of Montage Health, does not make grants to support individuals, endowment funds, or clinical research projects, nor do we make grants to organizations that discriminate in the hiring, advancement, or recruitment of employees or volunteers.


Submission Tips

  • You will need to create a free Submittable account or sign in with Google or Facebook credentials to submit to these forms.
  • You can save a draft of your work if you would like to finish filling out the form at a later date.
  • If anything changes with the information you submitted, please request to edit the submission. If you need to make changes to your form, please withdraw your submission and resubmit.
  • Submittable works best on Google Chrome, Firefox, and Safari. Internet Explorer is not supported. Please make sure you are using a supported browser.
  • We will follow up with you about your submission by email. Please be sure to safelist notification emails from Submittable and check the email you used to sign up for your Submittable account regularly.
